159th Convention Testimonial fo Change in Delegation Form

Purpose

This form certifies any changes to a congregation’s delegation for the 159th Convention of the Episcopal Diocese of Long Island. It must be completed if a delegate has resigneda replacement is being appointed, or an additional delegate is being added.


 

Instructions

  1. Download and complete the form linked below.

  2. Include the following:

    • Church name, town, and county

    • Name of the resigned or outgoing delegate (if applicable)

    • Replacement delegate’s name and contact info (if applicable)

    • Certification from the Rector, Priest in Charge, or Warden (if church is in vacancy)

  3. Submit the completed form by email to convention@dioceseli.org no later than November 7, 2025.


Requirements for Replacement Delegates

  • Must be a confirmed Communicant in good standing in the Episcopal Church.

  • Must reside within the Diocese of Long Island.

  • Must have been connected to the congregation for at least the past 12 months
