
Purpose
This form certifies any changes to a congregation’s delegation for the 159th Convention of the Episcopal Diocese of Long Island. It must be completed if a delegate has resigned, a replacement is being appointed, or an additional delegate is being added.
Instructions
Download and complete the form linked below.
Include the following:
Church name, town, and county
Name of the resigned or outgoing delegate (if applicable)
Replacement delegate’s name and contact info (if applicable)
Certification from the Rector, Priest in Charge, or Warden (if church is in vacancy)
Submit the completed form by email to convention@dioceseli.org no later than November 7, 2025.
Requirements for Replacement Delegates
Must be a confirmed Communicant in good standing in the Episcopal Church.
Must reside within the Diocese of Long Island.
Must have been connected to the congregation for at least the past 12 months
